Grant Thomson, MD *submitting author Introduction: This on going study evaluates the functional, psychological and social impact of hand trauma on previously healthy individuals. The study illustrates the clinical value of using psychometrically sound measures to screen for and identify those patients who may be experiencing rehabilitation difficulties, especially areas that may not typically be evaluated such as general health, body image and social concerns. Methods: Prospective enrollment began in July 2007 and currently 18 patients are enrolled and 12 patients have completed the study. Inclusion criteria include 18- to 75-year-old individuals who experienced hand trauma resulting in disfigurement within 3 months to 2 years prior to participation in the study.Ý The study includes photographs of the affected hand, demographic information and completion of psychometrically sound measures assessing upper extremity function (QuickDASH and HFS), quality of life (SF-36 and SWL), alcoholism (CAGE), depression (CES-D), post traumatic stress disorder (PCL ñC), body image (BIQLI, BICSI, MBSRQ) and social stigmatization (PSQ and ISEL-12). The results of the assessments were compared to those results attained from a community-dwelling control group. Results Preliminary analysis shows that patients with hand disfigurement experience statistically poorer general health as shown by the SF-36 scores (Mean Study 83±22, Mean Control 58±5, p=0.008). The results also showed increased reported confused staring behavior as shown by the PSQ score, Mean Study 2.2±0.9 Mean Control 1.2±0.4, p=0.014 Finally, the study group showed significantly decreased hand function as shown by the Function scale (Mean Study 39±21 Mean Control 25±0 p=0.008), DASH-disability (Mean Study 31±21Mean Control 3±4 p=0.003), and DASH-hobby (Mean Study 46±30 Mean Control 4±6 p=0.009). Conclusion Thus, in addition to functional deficits, there may be significant negative social and body image components related to hand disfigurement following trauma.
